How to Apply NHM, Staff Nurse, MLHP, ANM & Other Download Application: Application forms along with instructions can be downloaded from the official website of the respective District. For District Nagarkurnool, visit www.nagarkurnool.telangana.gov.in at District Notifications. Submission: Submit filled-in application forms in person at O/o DM&HO, Nagarkurnool District, IDOC building, Kollapur X Road, Nagarkurnool. Required Documents: Enclose self-attested copies of the following certificates with the application form: SSC or Date of Birth certificate Intermediate or 10+2 examination certificate Qualifying Examination pass certificate Marks Memo of all the years of qualifying examination Registration Certificates of respective councils Latest Community certificate /EWC certificate/Ex-service men / NCC / PH, whichever is applicable Study Certificate for the years from 1st class to 7th class or residence certificate for private study PH Certificate (if applicable) Relevant Certificate for Ex-Servicemen (if applicable) Registration certificate from the concerned Medical Council or Nursing Council or Para Medical Board / other relevant authority One Passport Size photo duly pasted on the application form Scrutiny and Merit List: After applications are received, they will be scrutinized, and a tentative merit list will be published on the district website for objections. The final selection list will be published after addressing all objections. Submission via Post: Applications sent via post should be addressed to the Office of the District Medical & Health Office, Nagarkurnool District located at IDOC Building, Kollapur X Road, with self-attested copies of required certificates.
