  • Registration certificates for Nurses and Doctors as applicable.
  • Proof of Tamil eligibility (10th or 12th standard mark sheet).
  • Community Certificate from Revenue Department.
  • Proof of residency (Nativity Certificate, Voter ID, Tax receipt, Aadhar Card, or Ration Card).
  • Character Certificate from a Government Officer (recent, within 3 months).
  • Character Certificate from the Head of Institution attended or currently attending.
  • If applicable, disability certificate from Block Medical Officer.
  • Certified work experience.
  • No Objection Certificate (if applicable).
  • Special records from competent authorities as per selection criteria.
  • Physical fitness certificate for camp life.
• Working Hours: Urban Primary Health Centre: 8:00 AM to 12:00 PM and 4:00 PM to 8:00 PM.
